ANNUAL
SAFETY REFRESHER
Those individuals already in our IQS system will receive a packet of paperwork that will need to be completed prior to participating in the Work Capacity Test. New guidelines require us to have the HEALTH SCREENING QUESTIONNAIRE form prior to the capacity test. The form will be reviewed and a letter of response will be sent to you, either stating you are cleared to participate in the pack test or it will provide further instructions. If the letter you receive from our office states you are all set, then you can RSVP for one of the pack tests and make sure you bring the letter. It will need to be presented to the Test coordinator on the day of your test. YOU WILL NOT BE ALLOWED TO TAKE THE PACK TEST IF YOU HAVEN’T RECEIVED THE “CLEARANCE” LETTER FROM US.
All those who are new to the program will need to start by having all the prerequisites in order. The following needs to be on file with our office:
· IS-700 certificate
· Three letters of reference:
From NH: 1st letter from your Warden
2nd letter from your Ranger
3rd letter from a person of you choice
Out of State:
You will still need to have three letters of reference. One of your letters of reference will need to come from that State’s Fire Supervisor.
Once your information is in IQS you will receive a packet with the necessary paperwork required to proceed.
